Students currently on STEM OPT that need to report a material change to their current I-983 such as

Change in Employer's Name
Change in Employer's EIN Number
Worksite Address Change
New Supervisor
Changes to employer commitments or student's learning objectives as documented on the current I-983
Reduction in compensation not related to a reduction in hours worked
Significant decrease in the number of hours engaged in STEM training opportunity
